    In East Africa, Uganda has the least prevalence of drug abuse at 16.7% (Illri, 2014). This is a great contributing factor worth looking out for in different communities. Thank you.

    Hi Denver,

    In our communities 90% of transmission is heterosexual with this means that the general population is highly affected. The PMTCT programme has been very successful in ensuring reducing the number of babies born with HIV. Currently our Ministry of Health is emplementing the option B plus which recommends for all HIV positive pregnant mothers to be initiated on ART.

    In my community this has decreased because there is more education on PMTCT by the department of health and other stakeholders. There are more activities taking place in the community.

    The uptake of PMTCT is very high. This has been because the primary health centers were empowered to proved prophylactic service to women who come to ante-natal clinic.

    We are experiencing the same problem im my community, there have been reports  of patients being mugged of their ARVs and some sell their antiretrovirals, from what we heard from the community is that they combine heroin and ARVs into a coctail and they get high,this coctail is called Nyaope or Whoonga. The biggest concerned here is the HIV virus drugs and resistence .

    no, in my community mother to child had no great up take. our community health centre did great job and they have started most female on ART’s to prevent mother to child.

    The Community Engagement team at FHI 360 fully understands the importance of stakeholder engagement. For instance, staff at FHI ensure there is community representation in every study from planning, implementation, recruitment, study close out, and result dissemination. Community Educators and CAB members are provided the opportunity to review concepts as they are drafted. Upon review educators and CAB members share their thoughts and how the proposed concept would impact their communities.

    Opposition of the Trial may arise due to social harms (Stigma or discrimination) that are determined to be associated with Trial participation

    Action; Research Team work closely with relevant Stakeholders in developing issues management plan prior to Trial implementation. This way Research teams are better equpped to deal with issues as they arise and are more likely to avert a crisis.

    Issue – Lack of recognition/understanding by the trial funder and/or trial sponsor on the importance of engagement with the community (thus, no interest or action from the start)

    Action – Training/education of these stakeholders to understand the essence of community engagement

    Our research team as constituted at the moment place priority on stakeholders engagement because of the country’s (Nigeria) experience with Pfizer litigation in the past.

    Our protocols are reviewed by the ethical review board before commencing any form of implementation.

    Relevant national stakeholders are officially communicated for administrative approvals and paid advocacy visits to brief them and share the protocol in acceptable format.

    The broader stakeholders which involve associations of medical doctors, nurses and laboratory personnel are engaged during their continue professional education (CPE) sessions.

    Community stakeholder engagement is based on two strategies namely demand creation which is focus on potential volunteers and conflict mitigation which deals with individuals and groups that could potentially oppose the trial. The media is involve from the beginning with key message development that is usually reviewed by a broad based Community Advisory Committee constituted for the specific research.

    The composition of the team for each assignment is based on team members who have competence, social connection and understanding of the working of the groups and individuals.

    Stake holders may feel left out in the process, this may prompt them to oppose you in the course of the study.

    Action- will be to send information on all that has been done, explaining every step that has been taken and reasons why. Through transparency, trust can be built.

    Hello everyone,

    I feel the lead research team has a good understanding of stakeholder engagement. This may not necessarily be so with the field teams  who form a large component of the study. At site or district level the site management team has a good understanding of stakeholder engagement as they are involved in planning and conducting of stakeholder meetings. Our CAB members are involved in advising study participants and resolving of incidents. This has helped in ‘marketing’ them.

    Lack of stakeholder’s consultation from the on start of planning the trial, failure to engage with local stakeholders that represent the community at large can have bad impact or lead to trials be opposed. Diversity of cultures in different communities need to be considered when conducting trials as community stakeholders posses knowledge and understanding of local cultures & perspectives and dynamics of the local HIV epidemic, Active community stakeholder’s engagement programmes can build sustained relationships between researchers and communities that is transparent and respectful, so the need for GPP work plan is of importance as that will minimise the unforseen challenges.

    I wonder if others have an experience of a CAB in Africa and how to keep members engaged. We also have a quite regular CAB recruitment but it would be great if we could retain the majority of members for a while (I think at my site is 2 years).
